The way I understand it, all you have to do is go to an "unpopulated" section of the astral plane (basically, one that hasn't been formed by another entity's thought yet), then do whatever you want with it. In these "virgin" areas, you're free to modify the landscape and laws of physics however you want. You're also free to set certain other rules (i.e. who can enter, whether you want it to remain after you leave, whether you want to use it as a meeting place for your guides, etc.), within limits. This is a commonly held metaphysical belief. I haven't APed successfully myself, so I haven't gotten a chance to try it, but almost every book I've ever read on AP says that the astral landscape can be pretty much shaped any way you like.

Yeah, except it wouldn't really be land. It'd be completely unformed and empty energy. Though you can modify the existing landscape if you want, it'd just be harder since something's already maintaining it.
